Please email a request to purchase a gift certificate, info@azmfr.com, or call (480) 594-1521.  Include in the email your first and last name, the recipient's first and last name, and the therapy or dollar amount you would like to gift. You will be emailed an invoice that will allow you to pay online. Payments can also be made by phone or in person.
 
You can either set up a pick-up time to get your certificate in person, it can be mailed to you or its recipient, or you can create something yourself to gift and the payment information will be held in the Arizona Myofascial Restoration system until the gift certificate has been redeemed.
