New Client Information

Welcome to Arizona Myofascial Restoration!  Please plan to arrive early for your first visit to allow plenty of time to find the office (directions below), and to fill out all necessary paperwork.  If you have a prescription, physician’s notes, notes from other therapies, imaging reports, or anything else you think might be helpful, please bring them with you.  Copies will be made with your kind permission. I ask that you do not wear any lotions or creams the day of your appointment, or that you wash them off prior to coming in as topical agents will have a negative affect on my ability to maintain proper contact with your tissue.  Many medications can affect the outcome of a session, so please inform me of any that you’re taking. Please also refrain from taking any pain killers or muscle relaxers within 4-6 hours of your appointment unless they are an absolute necessity. If you’ve recently received surgery or had a medical procedure, you’ll need to be cleared by your doctor before beginning therapy.  If you have scheduled a Structural Integration series, please remember to bring viewing clothes with you to each appointment (see What to Wear at the bottom of the Structural Integration page). Please do not wear any strong perfumes, colognes, essential oils, or arrive smelling like cigarette or marijuanna smoke or with strong body odors.

 

Cancellation Policy

Appointments cancelled after 8am the day before a scheduled appointment will incur a $35 cancellation fee.  Appointments cancelled after 8pm the day before a scheduled appointment as well as missed appointments will be charged the full price of the appointment.

Hydration

 

I recommend you bring your own water bottle.  For optimal results, it’s best if you arrive hydrated and continue to hydrate following each treatment.  With respect to the environment, bottled water is not provided, but we do have a water cooler in the office and encourage you to fill your own reusable bottle before and/or after each session.  If you happen to forget, paper cups are provided.

 

What to Expect for your First Visit

 

Many other types of therapy perform an “assessment,” treatment first and your therapy doesn't officially begin until your second appointment.  Therapy begins here at your first appointment, and much of my assessment is done by providing you with a full treatment.  What I feel through my hands is one of my most valuable assessment tools.

 

After filling out your intake form, I’ll quickly review it and we will have a thorough conversation about what brought you to Arizona Myofascial Restoration and what you’d like to get out the therapy.  You’ll then receive instructions about where to leave your things and how to lie on the table.  Then, once you’re ready, your bodywork session will begin. Verbal intake on average takes no more than five minutes, but we’ll talk until you’ve provided me with everything you think I should know and until I’ve collected all the information I need to give you a safe and effective treatment.  If you feel a longer conversation is necessary, you are more than welcome to contact me in advance, and we can talk over the phone or by email. Please keep in mind all appointments are scheduled for a set amount of time, and out of courtesy to the client booked after you, all appointments will end at their scheduled time.

Payment

All payments are due at the time of your service.  I accept cash, checks, and all major credit cards.  With a physician's referral, I accept HSA (Health Savings Accounts) and FSA (Flexible Spending Accounts) in credit card form.  If you do not have a card associated with your account, you will be required to use another method of payment at the time of your service, and I can provide you with detailed receipts to submit to your account administrator for reimbursement.  I do not accept insurance or Medicare.  Package deals are not available.

Treatment Recommendations

For clients coming in to resolve an issue or to reach a goal: most are not resolved or met with one treatment, so I encourage you to schedule four initial treatments, each approximately one week apart.  Each treatment builds on the last, and four will allow us enough time to see how well the therapy is working, and will guide us in developing an extended treatment plan. If we’ve seen no progress in four sessions, I will discontinue treatment.  If the therapy is working, I will most often recommend continued weekly treatments until we get you to about 80% of your goal. We’ll then start to space each session further apart until we reach the point that therapy is no longer needed. I do not give more than one treatment a week, and work under the philosophy that less is more.  More bodywork does not help you get better faster (a common misconception), but it can cause you to become dependent on it, and that’s not the goal here. Clients often ask me how many treatments it will take to fix their problem, and my unpopular answer is always, “I don’t know.” Every body’s tissue has its own story, and we’ve all arrived at this place of discomfort from a different path.  Plan on several sessions to effect lasting change, and know it is my ultimate goal to help you get well and release you from my care. No one therapy is right for everybody, so if my therapy doesn’t feel like the right fit for you or we hit a wall before we reach your goals, I have several other excellent therapies and therapists I can happily and confidently recommend.

 

For my pregnant clients:  because your body is ever changing, every day brings a new surprise.  If you’re experiencing a happy comfortable pregnancy, I recommend you come in once a month during your second and third trimesters and once a week during your last month.  If you are experiencing discomfort or pain, I recommend weekly treatments until your complaint is resolved. Because of the special hormones running through your system, your tissue is more adaptable than the average person, and musculoskeletal issues can usually be resolved very quickly.

 

Clients coming in for your own general well-being or on a maintenance schedule:  I recommend scheduling treatments once every 4-8 weeks.
